CONNECTED PARTNER AGREEMENT

Form Customer Authorization

The below-signed representative of the “Company” (as named below) hereby grants and authorizes Command Alkon Incorporated and its affiliates, subsidiaries or parent companies, as well as any of its employees, agents, and/or representatives (hereinafter jointly “Command Alkon”) the right to share the Company’s data from Command Alkon platforms utilized by the Company (including, for example, Command Cloud, TrackIt, etc.) with (“Command Alkon Integration Partner”) for the limited purpose of integration with and data population of Command Alkon Integration Partner platforms utilized by the Company, as well as allowing Command Alkon Integration Partner to  understand and consult with the Company to increase the value derived from the Command Alkon and Command Alkon Integration Partner platforms used by the Company. This sharing of data may include  personal data that may include, but is not limited to, data types including the following: names, addresses, telephone numbers, e-mail addresses, employee numbers, job titles, geo-location data, IP addresses, etc. The Company attests that it has the necessary legal basis to share such Personal Data with Command Alkon and Command Alkon Integration Partner.

This Customer Authorization is effective on the date signed by the Company. Command Alkon, Command Alkon Integration Partner, and the Company agree that they will each abide by all privacy legislation applicable to it and its access and use of the Company’s data. This Customer Authorization will continue indefinitely and may be revoked in writing at any time by sending a written request to privacy@commandalkon.com. Command Alkon will process and handle any such request without undue delay.

The Company and Integration Partner hereby releases and holds harmless Command Alkon from all liability, petitions, and causes of action which may arise from the sharing of data with Command Alkon Integration Partner as permitted herein.

The undersigned attests that he/she has the authority to execute this Customer Authorization on behalf of the Company. The undersigned has read this Customer Authorization before signing below and acknowledges that he/she fully understands the contents, meaning and impact of this Customer Authorization.
